Freckles vs. Melasma: How They Differ and How They Are Treated

Many patients tell us they use increasingly heavy makeup to cover freckles across their nose and cheeks. Because freckles contain sharply defined pigment in the superficial skin, they often fade well with lasers or light treatment. But the same energy can darken melasma on a face with both conditions. Identifying the pigment first is essential.

Why do freckles form?

The medical term for freckles is ephelides. A review of pigmentation research describes freckles as largely genetically determined, with sunlight bringing them out on the skin. Small brown spots often begin in childhood around the nose and cheekbones, darkening in summer and fading in winter.

The same review distinguishes lentigines, which are often casually called freckles. These sun spots reflect accumulated UV exposure and photoaging, enlarge with age, and do not fade seasonally. That is why we ask whether a spot has been present since childhood or appeared in recent years. A review of Asian skin notes that pigment changes often precede wrinkles, with melasma, lentigines, freckles, and post-inflammatory hyperpigmentation common.

How do freckles differ from melasma?

Their shape, location, and causes differ. These are the main distinctions among three common brown pigment conditions.

Freckles and lentigines often respond well to lasers. In melasma, pigment-producing cells are more reactive, so strong irritation can worsen discoloration. A meta-analysis of 22 laser studies found that most lasers reduced melasma severity scores, but darker skin carried risks of post-inflammatory hyperpigmentation and loss of pigment. Faint melasma is often mixed among freckles; overlooking it and treating everything aggressively is a major risk.

How are freckles treated?

The main options are pigment lasers and light treatment. A systematic review of 41 lentigo trials involving 3,234 participants reported success rates of 36.36–76.6% for Q-switched lasers, 67.9–93.02% for picosecond lasers, and 74.6–90% for IPL. These figures concern lentigines and cannot be applied directly to freckles, but offer context for treating epidermal pigmentation.

A few larger, well-defined spots often suit a laser that targets each lesion individually. Small spots scattered across the face may suit light treatment covering a broader area. We also consider Reepot and BBL when previous pigment treatment has been disappointing, because reassessing the pigment type may point to a different device.

Will one laser session be enough?

That depends on the thickness and number of spots. Higher-energy pigment treatment aims to remove the lesion in one session, but thicker or deeper lesions may need another. Light treatment generally fades pigmentation gradually over several visits, often about a month apart.

Sunlight brings freckles out. Removing current spots does not change the genetic tendency, so new ones can appear without consistent sun protection. Patients often ask whether spots recur. What appears to be recurrence can sometimes be a small lesion missed in the initial treatment plan or subtle dermal pigment.

How should you care for treated skin?

After pigment laser treatment, DuoDERM protects the site while new skin forms. If the dressing starts to lift, do not remove and replace it; apply a larger piece over it. After removal, residual redness returns toward your normal skin color over 1–6 months, depending on skin type. Avoid saunas and keep up moisturizing and sun protection during this period. We recommend waiting 1 month after removing DuoDERM before other pigment procedures such as toning. After light treatment, redness usually settles within hours and normal activities can resume.

What should you know before freckle treatment?

Pigment fading and the time redness takes to settle vary by skin type. Temporary pigmentation may develop where redness remains, then fade over time. Before a photo shoot or important event, allow for both the dressing period and residual redness. Light treatment is unsuitable with photosensitizing medicines, isotretinoin within the past 6–12 months, pregnancy, or recent tanning. We also generally postpone pigment lasers for autoimmune disease affecting the skin or during cancer treatment.
