Pico Toning vs. Laser Toning: What Is the Difference?
Patients often hear that pico toning is better than conventional laser toning. Both use low energy to break up melanin gradually; the main difference is pulse duration. Research does not show pico toning consistently outperforming conventional toning for melasma. Identifying the pigment matters more than the device name.
What is the difference?

Conventional laser toning usually uses a Q-switched Nd:YAG laser with pulses measured in nanoseconds (billionths of a second). Pico toning uses picosecond pulses, a thousand times shorter. When patients ask whether pico is better, we first establish which type of pigment is present.
Shorter pulses fragment pigment through a rapid mechanical effect rather than predominantly heating it. Picosecond lasers can produce a stronger mechanical effect at lower energy than nanosecond lasers. With a diffractive lens, they can also stimulate collagen and elastin for skin renewal.[1]
Comparing toning and targeted pigment removal
The distinction becomes clearer when we also include lasers used to remove individual pigment lesions.

Is pico toning better for melasma?
A split-face study of 20 patients compared a 755 nm picosecond laser with a 1064 nm Q-switched laser. At 6 months, only the Q-switched side had a statistically significant reduction from baseline, but the difference between the two lasers was not statistically significant. Q-switched treatment was less painful. At 2 years, 58.82% had recurrence or worsening, again without a difference between lasers.[2]
Melasma involves vessels, hormones, and UV exposure, so neither laser finishes the job in one session. Energy selection and how treatment is spread across visits matter more than the pico-versus-nanosecond label. We do not recommend high-energy Reepot removal for diffuse, easily irritated melasma. Instead, we plan an approach suited to melasma, such as HALO or BBL Forever.
Is laser toning safe?
A review of 42 studies found low-fluence Q-switched treatment generally effective and safe for melasma. It cautioned, however, that excessive cumulative energy can cause patchy hypopigmentation and overly aggressive treatment can provoke darker pigmentation through inflammation.[3] We therefore assess the response and adjust energy at every visit.
When is another laser more suitable?
Not every pigment lesion needs toning. Clearly defined lentigines, seborrheic keratoses, and dark freckles often respond more efficiently to targeted removal. A review of 41 lentigo trials found lasers more effective than other methods, with success rates of 36.36–76.6% for Q-switched and 67.9–93.02% for picosecond lasers.[4] We may therefore recommend targeted removal when previous pigment treatment has been disappointing.
Picosecond lasers also have uses beyond pigment. Creating microscopic responses in the dermis while preserving the epidermis can stimulate collagen, making them useful for acne scars and pores.
How does Dong-An Joongsim Clinic treat melasma, sun spots and freckles?

Among picosecond devices, Discovery PICO uses 532 nm and 1064 nm wavelengths for toning melasma, freckles, and other spots. It also treats acne scars and pores by stimulating the dermis while preserving the epidermis. Combining it with scar injections such as Core Pulling can improve scar results. What should you know before pico toning?
Melasma can darken again with UV exposure, hormones, or irritation and needs ongoing care. Session numbers, intervals, and improvement vary with pigment depth and skin tone. Strong, closely spaced treatments can cause pigment loss or darkening, so settings must follow the skin's response. After targeted spot removal, keep DuoDERM in place for the instructed period and wait 1 month after removal before toning or other pigment treatment.

Can melasma return after pico toning?
Yes. In the study, more than half of patients had recurrence or worsening at 2 years. Sun protection and maintenance care remain necessary.
Can pico toning remove freckles?
Light freckles may fade with toning. Dark, well-defined freckles and lentigines often need fewer sessions with a targeted laser such as Reepot.
