How Ultherapy Works to Lift Sagging Skin

Understanding how Ultherapy works helps set expectations while waiting for results. It focuses ultrasound at selected depths, creating small thermal points while leaving the surface intact. As these areas heal, tissue tightens and collagen forms. Results therefore build over months and last for different periods in different faces. We often see a good response when the cheeks and jawline have begun to sag or previous lifting results felt too brief.

How does Ultherapy lift sagging tissue?

Like a magnifying glass concentrating sunlight, this treatment focuses high-intensity ultrasound at a point beneath the skin. Tiny thermal coagulation points reach around 60–70°C, a range suited to collagen renewal, while the surface above remains intact.

A 2025 review explains that focused ultrasound creates localized thermal points in several skin layers and the SMAS. This stimulation recruits fibroblasts to form new collagen and elastin, improving firmness and texture and reducing laxity[1].

How deep does treatment reach?

Each transducer, the attachment at the handpiece tip, has a defined focal depth. Different depths address different layers and concerns.

The SMAS is also a layer lifted in facelift surgery. This distinguishes focused ultrasound from RF: RF mainly heats the dermis broadly, while focused ultrasound delivers focal heat down to the fascia. When do results appear?

Immediate contraction may produce a slight tightening sensation. Visible changes develop over weeks to months as collagen accumulates. A systematic review of 16 facial studies found clinician-assessed improvement in firmness or wrinkles in 92% at day 90, maintained up to a year. Patients described the degree of improvement as mild, but the proportion reporting it rose from 42% at day 90 to 53% at day 360[2]. Change generally becomes clearer gradually rather than appearing all at once. One first-time PRIME patient felt their jawline lifting after a few days. For a photo shoot or event, we therefore work backward from the date and recommend treatment well in advance.

How long do results last?

Duration depends on age, thickness, laxity, and line count. The same review noted a lack of follow-up beyond a year[2]. Reassessment is more useful than fixing the next date in advance. Another option is to combine Thermage FLX, which heats the dermis broadly, to address two layers. Patients seeking firmness and contour together often receive both devices. One reported clearly improved elasticity and better definition beneath the chin and along facial contours.

Why does real-time imaging matter?

Thickness varies between people and between areas such as the forehead and jawline. A focus that is too shallow or too deep can reduce benefit and increase discomfort. Expert consensus comparing ultrasound devices concluded that live layer visualization and coupling detection help prevent complications and improve safety and effectiveness[3]. The forehead and jawline should not simply receive identical depth and line counts. Adequate treatment at each required layer matters more than an indiscriminately high total.

What should you know before ultrasound lifting?

Ultherapy works best for mild to moderate laxity. The review considered severe sagging or BMI above 30 relative contraindications because results were poorer[2]. Bone-like aching may occur during treatment, followed by swelling or soreness for 1–2 days. Other effects such as altered sensation or bruising were uncommon, at 2%[2]. We do not recommend this treatment alone for severe sagging when it is unlikely to meet expectations. Avoid hot saunas, heat treatments, and strenuous exercise for a week afterward.
