Eye Wrinkles and Drooping Eyelids: Can Nonsurgical Treatment Help?

Eye wrinkles are difficult to conceal with makeup, and drooping eyelids can make the eyes look smaller and the face tired. Mild wrinkles and early eyelid laxity respond well to nonsurgical treatment. The right approach depends on whether the cause is loose skin or weakness in the muscle that opens the eye.

Why do eye wrinkles form?

Wrinkles develop as the dermis thins and the expression muscles beneath it contract repeatedly. A review in American Family Physician identifies these two factors (dermal atrophy and repeated muscle contraction) as the main causes. The skin around the eyes folds every time we blink or smile, so lines often appear there early.

Eye-area skin is among the thinnest on the face and has few oil glands, making it prone to dryness. As collagen declines, fine lines remain even when you are not smiling, and loose upper-eyelid skin begins to cover the eyelid crease.

How do wrinkles and loose eyelid skin differ from ptosis?

They can look similar but have different causes. Loose skin may hang toward the lashes; true eyelid ptosis occurs when weakness in the lifting muscle lets the lid cover more of the eye. A lowered brow can also press down on the eyelid. Telling these apart is our first step, because skin tightening alone will not deliver the expected change in someone with true ptosis. We check how strongly your eyelid opens so this is not overlooked.

Each eye-area concern has its own cause and treatment approach.

Are injections enough for eye wrinkles?

Muscle-relaxing injections work well for lines that appear only when smiling. According to the same review, botulinum toxin takes about 2 weeks to reach its full effect and lasts 3–4 months, with a clearer effect on movement-related lines than on wrinkles at rest. Established fine lines and loose eyelid skin may also need collagen-stimulating treatment.

Can Eye Thermage improve wrinkles and eyelid laxity?

Eye Thermage delivers radiofrequency through a dedicated 0.25 cm² tip around the eyes and on the eyelids. In a 2025 study, 14 Asian participants received one upper-eyelid treatment with a monopolar RF tip of this size. Improvement greater than 50% was reported by 73% at 2 months and 86% at 6 months. The central brow rose 1.45 mm at 6 months, with no adverse effects observed. Protective eye shields are worn during treatment.

A multicenter study also found significantly fewer wrinkles at both 4 and 16 weeks after one treatment of the eye area and cheeks with a 5 cm² monopolar RF tip. Thermage FLX measures skin impedance with every pulse and automatically adjusts the energy. It can treat the forehead, the eye area, and under the eyes. It often suits people who have lost firmness after rapid weight loss or prefer a nonsurgical approach to wrinkles and laxity. Patients sometimes tell us they have heard Thermage does not work. This often comes from judging the result too soon; we recommend comparing photographs after 2–3 months, once new collagen has formed.

Is Sofwave suitable for forehead and eye-area lines?

Sofwave delivers parallel ultrasound to the mid-dermis at approximately 1.5 mm to stimulate new collagen and elastin. A 2023 review reported meaningful improvements in brow lifting and fine lines with FDA-cleared Ultherapy and Sofwave. This ultrasound treatment can address forehead and eye-area lines together. Because it has little effect on pigment, it is also used on darker skin. It can suit people who prefer to avoid surgery or threads. We recommend waiting at least 48 hours after filler or botulinum toxin injections, and at least 3 months between additional sessions, depending on skin condition.

Radiofrequency is not suitable for people with implanted electrical devices such as pacemakers, so we ask about these before treatment.

We use numbing cream for both Thermage FLX and Sofwave. Thermage produces brief heat with each pulse. Sofwave can feel hot and prickly, particularly near bone such as the forehead. Sensitivity varies.

Can I resume normal activities after eyelid treatment?

Yes, after either Thermage FLX or Sofwave. Redness and swelling usually settle within hours and at most within 1–2 days. Avoid saunas for about a week.

What determines the cost of Eye Thermage?

Cost depends on pulse count and whether treatment covers only the eye area or the whole face as well. We explain the cost after your doctor's assessment.
