Hollow Cheeks After Lifting: Why They Happen and How to Reduce the Risk

Worry about hollow cheeks makes many people hesitate before lifting treatment. Hollowing usually depends less on the device itself than on whether the treatment depth and areas match how fat is distributed in your face. The most useful precaution is to assess skin thickness and fat volume beforehand, then skip lean areas or treat them more gently.

Why can cheeks look hollow after lifting?

Lifting devices use heat to stimulate collagen; with some devices, that heat also reaches fat. A 2025 review of high-intensity focused ultrasound (HIFU) explains that ultrasound-induced heat both stimulates collagen renewal and reduces fat-cell size. This can slim a fuller face, but concentrating heat beneath the cheekbones or in the lateral cheeks where fat is already sparse can leave a hollow.

The same review notes that complications can occur even with minimally invasive HIFU. In many cases, hollowing reflects a treatment plan that did not fit the face, rather than the device itself.

Is lifting always the cause?

No. A 2024 review of facial aging describes fat loss in specific compartments as a major cause of structural aging. The cheeks may already be losing volume gradually; lifting the sagging tissue can simply make those existing hollows easier to see.

Weight changes also matter. A 2025 imaging study of patients taking weight-loss medication found that midface volume fell by approximately 7% for every 10 kg lost, mainly in the superficial fat layer. We plan more cautiously when a patient is dieting or has recently lost substantial weight.

Common causes of cheek hollowing

How can hollowing after Ultherapy be avoided?

Ultherapy sends ultrasound energy as deep as the SMAS, so selecting the depth matters. Is ONDA suitable for a face with little fat?

ONDA delivers 2.45 GHz microwave heat from the dermis down to the fat layer. The shallow 3 mm handpiece contracts collagen, while the deeper 7 mm handpiece promotes fat-cell breakdown. This is useful for double chins and sagging cheeks, but lean faces need a plan built around the shallow handpiece. When patients say the treatment did little, the same settings have often been used repeatedly without assessing fat thickness and sagging, and hollowing can result from the same oversight.
