Neck Lines: Why They Form and What Treatment Can Do

People often start caring for their neck later than their face, and established lines rarely fade much with cream alone. Complete removal is difficult, but appropriate treatment can visibly soften lines and improve firmness. We first establish whether the concern is a horizontal crease, thinning skin, or laxity.

Why do neck lines form?

Neck skin is thinner than facial skin and has fewer oil glands, so it dries easily. Looking down repeatedly folds the same areas. As dermal collagen and elastic fibers decline, the folds no longer fully smooth out. Long-term UV exposure makes skin thinner and rougher. Prominent platysma muscles at the front of the neck can also create vertical bands. Patients often ask whether a different cream will remove neck lines; established creases reflect lost dermal collagen, which topical care alone cannot fully replace.

Does the type of neck line change treatment?

The visible pattern and cause determine the main approach.

A 2023 review of nonsurgical neck treatment recommends a similar approach: botulinum toxin for prominent muscle bands with little skin laxity, and focused ultrasound or RF for mild to moderate laxity.

How much can treatment improve neck lines?

The aim is to rebuild support so skin folds less deeply, rather than erase every line. In a 2017 study of diluted CaHA in the neck and chest, biopsies showed increased type I collagen at both 4 and 7 months, with improved ultrasound-measured dermal thickness and elasticity. In a 2023 randomized trial, three HA skin-booster sessions reduced fine neck lines more clearly than cream alone.

What treatments are available?

Injectables include hydrating skin boosters and collagen stimulators. A 2025 review reported improved facial and neck wrinkles, elasticity, and hydration with PLLA, with satisfaction above 90%. Device options include RF to heat and tighten the dermis, focused ultrasound reaching the fascia, and lasers that heat the dermis evenly. Deep lines with loose skin often call for injections and devices together. Sculptra can suit patients who are also concerned about aging hands or lost firmness after weight loss, because the same stimulator can be used on the face, neck, and hands. We do not recommend it for people with a history of keloids or hypertrophic scars, connective-tissue disease, or active immune disease.

How should you care for your neck at home?

Extend sunscreen and moisturizer down to your neck. Spend less time looking down at your phone and avoid an overly high pillow. These habits help slow new lines; established deep creases respond more quickly to treatment that restores support.

Some patients ask for a high pulse count to finish in one visit, but more is not always better. One patient said they appreciated being advised on a pulse count suited to them.

What should you know before treatment?

Thin neck skin generally looks more natural when volume is built over several sessions rather than added all at once. Collagen stimulators take weeks to months to work, and results vary between individuals. Sculptra protocols use sessions 45 days apart, so judge the result several months after completing the course. Similarly, compare RF tightening results after 2–3 months of collagen formation rather than judging immediately. Bruising, swelling, or small lumps may occur after injection but usually settle within days. Tell us if you have an implanted electrical device such as a pacemaker, since this rules out RF treatment.
