Under-Eye Hollows and Dark Circles: Is Adding Volume Enough?

Under-eye hollows can make you look tired even after enough sleep. Filler may seem an obvious answer, but this area has very thin skin and complex anatomy. Understanding why it is hollow matters more than deciding what or how much to inject. Separating the causes of hollowing and dark circles helps us combine volume restoration with skin support for a natural result.

Why do under-eye hollows develop?

Lower-eyelid skin is among the thinnest on the face. Age-related loss of dermal collagen and HA makes it thinner still. At the same time, supporting ligaments loosen and cheek fat descends, accentuating the boundary between the lower eyelid and cheek: the tear trough.

A 2023 review notes that tear-trough hollowing often coexists with malar mounds, eye bags, and dark circles. It emphasizes restoring midface volume as well as the under-eye area to improve both local results and the overall face. That is why we also assess the front of the cheek.

Are dark circles and under-eye hollows the same?

No. A systematic review groups dark circles into pigmentary, structural, vascular, and mixed causes. Hollowing is a structural cause: a shadow makes the area look dark. Different causes respond to different treatments, so identifying them comes first.

The review concluded that filler and fat transfer help volume-related dark circles, while surgery is more effective for substantial excess skin.

Will filler alone solve a hollow?

Filling can help a clear depression. A systematic review of under-eye filler reported high satisfaction and effects lasting 8–12 months. Thin skin, however, makes bruising, swelling, bluish discoloration known as the Tyndall effect, and unevenness possible.

Problems may also appear later. An analysis of 52 delayed under-eye filler complications found swelling in 42.3% and lumps in 25.0%, with an average onset of 16.8 months. This is why we use small, evenly distributed amounts rather than trying to fill the entire hollow at once. When patients ask whether filler can solve everything, these limitations inform our conservative approach.

What role do skin boosters play?

When thin skin contributes to darkness, improving skin thickness and firmness may matter more than adding volume. HILO WAVE places a high-/low-molecular-weight HA complex just beneath the skin to replenish hydration and encourage collagen. As an HA-based product, it can be dissolved with an enzyme if nodules form. It may suit people concerned about nodules, a foreign-body sensation, or recovery time. For pain-sensitive patients or those with an event coming up soon, we select a cannula approach with minimal downtime. The manufacturer advises against use around the eyes, including the glabella, because intravascular injection can cause serious harm such as blindness. Your doctor therefore assesses thickness and vessel position, selects appropriate areas and amounts, and avoids areas close to the eyes.

Thin, lined under-eye skin: how does Re2O support skin structure?

We consider Re2O when under-eye skin is thin and lined. It introduces purified human-derived extracellular matrix into the dermis to reinforce the structure supporting collagen and elastin. It can suit people with many fine lines, rapid loss of firmness, or reluctance to repeat filler. Changes become clearer at 2–4 weeks; the usual starting course is three sessions 4 weeks apart. Thin areas such as the under-eyes require reduced depth and volume.

What should you know beforehand?

We do not recommend injections first for markedly protruding fat or substantial excess skin; surgery may be more appropriate. Mild redness or swelling may last 1–3 days.
